Nakama has seen a very soft side of Ichigo because of circumstances, I think, so she'd be surprised to know that he can be surly and violent (beyond attacking ice fairies, of course). She doesn't quite think of him as gentle, but he seems to consider the feelings of others and share her reckless desire to help without much concern for his own safety. Also, while he checked that she was alright, he didn't give her shit for being out or question her in any way about trying to get to the hospital in that mess, which she respected deeply. He'll protect her but still trusts her, that's what it said to her.

If she ever catches him being surly, though, he'll get the full brunt of her bossiness about manners. There's no dodging that, Ichigo!
